## Key Conventions

- `db.flush()` in repositories, not `commit()`
- Services raise `NotFoundError`, `AlreadyExistsError`
- Separate `Create`, `Update`, `Response` schemas
- Commands auto-discovered from `app/commands/`
- Document ingestion via CLI and API upload
- Sync sources: configurable connectors with scheduled sync
